Please arrive 20 minutes prior to your initial appointment and allow extra time if you plan to complete the forms when you arrive. You will NOT be seen with incomplete paperwork.
You will receive a reminder phone call about your appointment 1 week ahead of time. (PLEASE LISTEN TO THE OPTIONS BECAUSE A RESPONSE IS REQUIRED.)
If you are unable to keep your appointment, we kindly ask for a 24 hour notice. This allows us to offer that time slot to others who may be waiting for an opening.
